   (Please consult the Erasmus Office regarding this matter.)

6. Traineeship Host Institution Approval Form

7. Erasmus Voluntary Traineeship Application Petition

8. OLS Certificate
   (Language proficiency test to be completed via EU Academy)

9. Visa Copy

B. Documents to be submitted after the mobility for Erasmus PhD Traineeship Mobility

1. Certificate of Participation

2. After the Mobility Document

3. Evaluation Form

4. Passport Entry/Exit Pages
   (or Entry/Exit Certificate obtained via e-Government)

5. Traineeship Report

6. Erasmus Participant Report (To be completed via the link sent by email after the mobility period ends.)
